Hello, my name is Ram. My animal id is #17013. I am a desexed male gray cat at the Manhattan Animal Care Center. The shelter thinks I am about 10 years old. – P
I came into the shelter as a aco impound on 27-Dec-2017, with the surrender reason stated as person health – going into care facility.
Ram is geriatric cat who is at risk for medical reasons. Ram has been diagnosed with Upper Respiratory Infection, which is contagious to other cats, and also has a number of other underlying conditions, including, hyperthyroidism with muscle wasting, dehydration and conjunctivitis. Behaviorally, we have no concerns for Ram, whom we believe would be suitable even for beginner cat homes.

[DVM Intake] DVM Intake Exam Estimated age: 10yrs Microchip noted on Intake? none Microchip Number (If Applicable): History : found inside apt of someone who was taken to a hospital Subjective: offered food- didn’t show interest Observed Behavior – purring, allows all handling, friendly Evidence of Cruelty seen -no Evidence of Trauma seen -no Objective P = 180 R = purring, but eupneic BCS EENT: Eyes clear, ears clean, no nasal discharge, some mild dried debris ventral to OU. Eyes somewhat sunken, no palpable thyroid nodule, slightly tacky mucous membranes Oral Exam: halitosis, no obvious oral masses or lesions- p resents oral cavity being examined, very difficult to open mouth, unable to fully assess PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MC MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, mild overall muscle wasting CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment MC approx 3-5% mild dehydration (r/o secondary to underlying chronic condition, secondary to starvation, decreased intake, other) mild diffuse mm wasting: r/o secondary to aging, underlying metabolic/endocrine unsure of appetite at this point Prognosis: open pending diagnostics Plan: LRS 75ml SQ given, will con’t SID x3 more days cerenia 10mg/ml: 0.4ml SQ given in case of inappentence/nausea check app requested cbc/chem/t4 alerted NH about p SURGERY: N/A- p neutered
Hx: geriatric, underweight on intake; CBC/chem/T4 run today S/O -QAR, docile, allows all handling -mm pk/lgt pk, tacky; CRT <2 sec; dental disease -mild mucoid nasal discharge from L nares -OU: eyes appear sunken, mild to moderate blepharospasm, green mucoid discharge -AS: previous ear tipped -eupnic, no murmurs but tachycardic (HR 180) -soft, doughy abdomen with no palpable abnormalities -moderate to severe diffuse muscle wasting A 1. URI/conjunctivitis 2. Dehydration 3. Hyperthyroidism 4. Elevated ALP, GGT-r/o secondary to hyperthyroidism vs pancreatitis vs liver dz vs other 5. Low BUN/creat-r/o inappetance vs muscle wasting vs liver dz 6. Tachycardia-suspect secondary to hyperthyroidism P -Place IVC, run LRS with 20 mEq/L KCL at 15 ml/hr -Cerenia 0.3 ml IV SID x 3 days -doxycycline 0.7 ml PO SID x 10 days -Terramycin OU BID x 7 days -Methimazole 2.5 mg PO BID, recommend rechecking T4 and renal values in 2 weeks
Recheck in med-iso geriatric cat with hx of hyperthyroidism, poor BCS/dehydration, URI/conjunctivitis: S/O: Quiet, resting in kennel, perks up with PE/allows exam – drinking/urinating – did not see eat dry or wet food yet – mild purulent nasal discharge, OU mild mucoid ocular discharge; mod. dental disease – h/l nsf (hx of tachycardia) – amb x 4, BCS 4/9 with gen. muscle wasting, unkempt coat; soft abdomen A: hyperthyroidism with muscle wasting/dehydration; URI/conjunctivitis P: CWCT, monitor appetite
Recheck in med-iso geriatric cat with hx of hyperthyroidism, poor BCS/dehydration, URI/conjunctivitis: S/O: BAR this AM, vocal and coming to the front of the kennel – eating, drinking/urinating – mild purulent nasal discharge, OU mild mucoid ocular discharge, face was cleaned – h/l nsf (hx of tachycardia) – amb x 4, BCS 4/9 with gen. muscle wasting, unkempt coat; soft abdomen A: appears brighter today, + hyperthyroidism with muscle wasting/dehydration + URI/conjunctivitis P: CWCT, monitor appetite
Recheck in med-iso geriatric cat with hx of hyperthyroidism, poor BCS/dehydration, URI/conjunctivitis: – IVF LRS @ 15 ml/hr S/O: BAR, mildly dehydrated, vocal and coming to the front of the kennel – eating, drinking/urinating – no nasal discharge this observed this morning; OD mild mucoid ocular discharge, face was cleaned A: URI/conjunctivitis much improved + hyperthyroidism with muscle wasting/dehydration P: CWCT

22g RFL IVC placed, running at 15mL/hr. 20 mEq kcl added to 1 Liter bag of LRS. Gave 0.3mL Cerenia IV.
Given 75ml LRS SQ and 0.3ml Cerenia SQ
Pulled IVC today per DVM 1088

Behavior Condition: 1. Green
Cat allows some handling
KNOWN HISTORY:: Ram was brought in as a stray so we cannot speak to his behavior in his previous home.
MEDICAL BEHAVIOR:: Observed Behavior – purring, allows all handling, friendly
ENRICHMENT NOTES:: Dehydrated, not eating. Resting in litter box, soft eyes and body, seemed a little rundown. Lay in place when door opened, leaned into pets, purred.
Cage Condition:: Cage is neat
Reaction to assessor:: Ram immediately comes to the front of the kennel, looks at the assessor with soft eyes and body.
Reaction when softly spoken to:: Ram begins to rub against the bars.
Reaction to cage door opening:: Ram remains calm and relaxed.
Reaction to touch:: Ram head-butts the assessor’s hand, leans and arches into pets, and purrs.
Reaction to being picked up:: Ram puts his paws over the assessor’s shoulder and sits calmly.
ACTIVITY LEVEL:: Mellow
VOCAL:: Quiet
