Medical Summary DVM Intake Exam 5/20/17 Estimated age: ~4-5weeks, female Microchip noted on Intake? No History : Stray kitten, presented with one other kitten Subjective: Observed Behavior – Q/BARH. Hissing, allowed all handling. Eating well on own Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective P = WNL R = WNL BCS 3/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Pale pink mm, eyes clear, no ocular or nasal discharge PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Underage, underweight Plan: Continue to monitor while at BACC. Gave FVRCP RF Prognosis: Good SURGERY: Temporary waiver due to underweight/underage
