Medical Summary DVM Intake Exam Intact male Estimated age: 2.5 months Microchip noted on Intake? negative History : Patient is stable. Stray kitten. Subjective: Apparently healthy male kitten. Observed Behavior – Hissing, trying to flee, Evidence of Cruelty seen – NAF Evidence of Trauma seen – None Objective P = 180 bpm BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: NAF PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment Apparently healthy Plan No treatment for the moment Prognosis: excellent

Intake exam 7/23 – NSF. Housed with A1119456 S/O) Very dull and weak. Unable to stand but lifted head some. Crying out when handled but limp. T= 93. Slight dehydration – 3%. MM pale. MPOD OU. NND. Ulcer noted on L upper lip and R lower lip. Small ulcer seen R upper lip. Pinpoint ulcer on hard palate. No ulcers on tongue. CV: HR= 180, 2/6 murmur L: clr bilat. Abd: SNP, NMP Gave 25 cc SQF PCV=5%, TS = 4.3 Parvo test – negative. (Very little BM in rectum.) A) Severe anemia. Conjunctivitis and oral ulcers – suspect calici. Declining P) EHR. Place litter mate on vet check to monitor.
