BAR-H, MMs pink and moist, BCS 7/9
EENT: Moist nose, not dripping. Eyes clear, ears clean, no ocular discharge noted.
Oral Exam: Mild tartar.
PLN: No enlargements noted
H/L: End-systolic heart murmur best heard L and R parasternal. Tachycardia. NSR. Lungs clear, eupnic
ABD: Soft, non painful, no masses palpated
U/G: Male neutered
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally
1. Heart murmur, tachycardia – R/O HCM vs. other heart disease vs. incidental murmur
3. Moist nose – R/O early URI vs. normal for this pt
4. Dental disease
1. Recommend weight loss of 2-3.5 lbs
2. Blood pressure, CBC/chemistry/T4, urinalysis, recommend echocardiogram
3. Recommend daily tooth brushing and regular dental cleanings
