sneezing, mpod As per vet. set up on doxy 50mg/ml 1.5cc sid x 10 days received convenia on 10/5 10/8/16 hiding in box but able to visualize mouth. No visible wounds/bleeding/drooling APPT (+) P: cont pain meds as prescribed 10/06/16 13:28 collar stuck in mouth! did not see this yesterday and I feel I would have seen it when cat was hissing, but possible collar has been there 24 hours. punchi was friendly and allowed chin scratched once collar removed. continue pain meds recheck tomorrow 10/05/16 17:25 hard to examine–hissing, in feral den open mouth, ptyalism. no other signs of uri but oral ulcers suspected move to URI ward convenia 0.5ml buprenrphine .3ml bid recheck tomorrow in URI ward 9/29 Was here from 06/2010-07/2010 Neutered male Scan positive for a microchip #985121007177762 6 years old according to the owner OU=clear Nose= No discharge AU=waxy Teeth are stainned. Upper right k9 is chipped Coat is clean Dewormed with Pyrantel Flea Treatment= Activyl As of now no signs of URI As of now no V/S/C/D NOSF Was very nervous during the intial and all he wanted to do was flee. 09/29/16 10:41 Basic Information Punchi is a 6 year old neutered domestic short hair who is being surrendered because owner is moving out of the country and cannot take. Owner adopted Punchi from ACC about five years ago. Owner stated Punchi has no medical issues and last saw a vet 3 years ago. Socialization Punchi is shy for a few minutes and then very affectionate around strangers. He has spent time with a 9 year old child and was very affectionate with the kid. He plays in a gentle manner with kids and adults. He lived with a 6 year old male cat. They were very affectionate towards each other and played in a gentle manner. He has never lived or spent time with a dog or any other kind of animal. He has never scratched or bit a person or animal. Behavior Owner stated Punchi has no behavior issues and slept during the car ride to our shelter. Owner stated Punchi is not bothered if bathed, by having his nails trimmed, being brushed, being picked up, placed inside of a carrier, or if disturbed while sleeping. For a new Family to Know Punchi is described as friendly, affectionate, playful and shy sometimes, with a medium activity level. He tends to follow his owner around when home. He likes to play with balls, string, laser pointers, and catnip toys. He is kept mostly indoors and sleeps on owner’s bed. He eats a half can of Friskies wet food a day and owner leaves Friskies dry food out. He uses a hooded cat litter box with clumping style litter. He has a scratching post made out of wood with bark. Behavior during Intake Punchi tolerated all handling during intake. He purred and arched his back when I petted him. No signs of aggression were displayed during intake.

Punchi’s former owner stated he is shy for a few minutes and then very affectionate around strangers. He has spent time with a 9 year old child and was very affectionate with the kid. He plays in a gentle manner with kids and adults. He lived with a 6 year old male cat. They were very affectionate towards each other and played in a gentle manner. Also, described her as friendly, affectionate, playful and shy sometimes, with a medium activity level. Reaction to assessor: Punchi looks tense, hiding in the cat den when approached by the assessor. Reaction to door opening: Punchi remains crouched down in place, wary with his surroundings, alert with eyes wide open. Reaction to touch: Punchi was unsure at first then slowly began to lean into head-butts the assessor’s hand and appreciates petting on the head and body. Reaction to Being Picked up: He becomes antsy then jumps back into the cage. Placement determination: Average Punchi interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.
