Medical Summary DVM Intake Exam Estimated age: 4 weeks old Microchip noted on Intake? none History : reported to be eating on own- offered canned to face, eating Subjective: Observed Behavior – allows all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = P = 160 R = 24 BCS 5/9 EENT: OU mild purulent d/c, ears clean, mild purulent nasal d/c Oral Exam: pink gums, deciduous dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI ,grossly normal MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment URI with conjunctivitis Plan move to medical iso doxycyline 50mg/ml: 0.1ml PO SID x10days erythromycin OU BID x10days Prognosis: good SURGERY: Temporary waiver due to underage, underweight, URI
