02/22/2017 Exam Type CAGE EXAM – Medical Rating is 1 – NORMAL , Behavior Rating is NONE, Weight 4.5 LBS.
Hx: The patient is stable. No vomiting, diarrhea, coughing or sneezing. Stable. Seem to be breathing weel. Alert and responsive but not reactive. O Seem to be euhydrated. BCS 5/9 EENT- No abnormal discharge from any system. Skin- No obvious ectoparasite seen. Cv- Could not auscultate. Ms- NAF Neuro- Ambulatory in all 4 Lymph nodes- Normal size and shape. Abdomen- NAF UG- Could not examine the UG system. A: Gingivitis Plan No treatment at this moment.

02/20/17 13:24 Finder did not have any interaction with animal as it was in a trap. Behavior during intake: Animal was fractious in a trap and trying to flee. Counselor was able to transfer from trap to den without any issues. Once in the den, animal kept trying to flee. No other handling was done.

Upon intake, Provolone attempted to flee and was not handled. EVALUATION: Cage Condition: Cage is neat. Reaction to assessor: Provolone is hiding in his den. Reaction when softly spoken to: Provolone continues to hide. Reaction to cage door opening: Provolone starts searching for an exit, actively avoiding assessor. Reaction to touch: Provolone avoids touch and attempts to flee, not fractious. MEDICAL BEHAVIOR: nervous, tense, attempted to flee ACTIVITY LEVEL: Lively VOCAL: Quiet CHARACTER TYPE: Shy, Skittish RECOMMENDATIONS: _x_ Placement with a New Hope partner POTENTIAL CHALLENGES: _x_ Fearful Behavior observations _x_ New Hope Only Provolone is displaying behaviors that preclude placement in the adoptions room and/or may require further investigation before placement in a home. He is extremely fearful in the shelter environment and does not currently tolerate petting or handling. The behavior department feels that placement with a New Hope Partner is the best option at this time.
