SAFE 1/12/18 PRINCESS lost her home because her owner died. She needs some dental care and a new home asap!

Medical Behavior: Blue

Age: 4 years

Sex: Female

Weight: 9 lbs

DVM Intake Exam

Estimated age: 3-6 years based on overall appearance and dentition
Microchip noted on Intake? Scanned neg on intake – MC placed during LVT exam

History : Surrendered after owner passed away, no health hx available

Subjective: Alert, looks around exam table.

Observed Behavior – Crouched to the table, eyes dilated. Allows all handling, soft body.

Objective
BAR-H, MMs pink and moist, BCS 6/9
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: Moderate tartar on L and R maxillary PM4s; other teeth have little tartar
PLN: No enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Female, no spay scar palpated
MSI: Small amount of dried blood on LH foot near last toe (5th toe). The nail is very short. Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally

Assessment:
1. Toenail ripped last night
2. Dental disease
3. Presumed intact

Prognosis: Good

Plan: Recommend dental cleaning and daily tooth brushing.

SURGERY:
Okay for surgery
