Medical Summary DVM Intake Exam Estimated age: 4-5 years Microchip noted on Intake? negative History : owner surrender Subjective: BARH Observed Behavior -growled and hissed and thrashed around during PE and medical procedures. Did not try to bite but was very difficult to handle and also difficult to get back in the carrier. Able to handle with a towel and no sedation Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = wnl BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: clean adult dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MN MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Apparently healthy Plan: CTM while at BACC Prognosis: excellent SURGERY: neutered
