SAFE 9/6/18 – PIXIE needs follow up medical care! @ BACC. Owner Surrender who is likely in hepatic lipidosis not eating and is yellow. She needs to be sent to a hospital ASAP for a feeding tube, IVF, AUS, BW, and supportive care.
PIXIE was adopted back in July and was sadly returned. She is spayed and ready to go to a new loving home.

History : Original intake 7/6/18-obese with dental disease. 9/1-owner surrender reported ot be losing weight. Gave SQF, cerenia, clavamox, and metronidazole. Started nebulization.
CBC: mild anemia (Hct 28.9), mild neutrophilia (13.62)
Chem: Mild to moderate elevation in ALT (236) with mild to moderate elevation in GGT (7) and moderate to severe elevation in ALKP (469) and moderate to severe hyperbilirubinemia (6.8)
Subjective: QARH. Allowed all handling. Not interested in food. No csvd.

U/G: FS, no MGTs, no vulvar d/c
MSI: Ambulatory x 4, skin free of parasites, no masses noted, severely icteric pinna, guma, sclera, and overall skin
CNS: Mentation appropriate – no signs of neurologic abnormalities

1) Elevated LES/hyperbilirubinemnia- r/o pre-hepatic (less likely- hemolysis from mycoplasma or IMHA) vs hepatic (cholangiohepatitis v lymphocytic portal hepatitis v hepatic lipidosis v neoplasia) v post-hepatic (mucocele, choleliths, cholangitis, neoplasia)
2) Dehydration-improved
3) URI
Prognosis: guarded longterm prognosis
Plan:
Extend LRS 100mls SQ SID until 9/7
Extend maropitant 1 mg/kg SQ SID until 9/7
Continue Clavamox 13.75 mg/kg PO BID until 9/15
Continue metronidazole 10 mg/kg PO BID until 9/8
Start syringe feeding BID (scheduled until 9/8)
Seek placement ASAP- needs further supportive care (fluids, enteral support), AUS, e-tube
