STRAY INTAKE: 1/29/2017
Initial Exam (1/29/17)
Estimated age: 17 based on overall condition – known to be at least 15 years old
Fleas: No flea dirt.
S: Quiet but alert, allows all handling, head-butts the examiner’s hand. Very stressed out by clippers and needed to rest after they were used to remove mats
O: QAR, decreased skin turgor, estimated 10% dehydrated, BCS 1/9, MMs very pale
EENT: Moderate mucoid discharge OU, enophthalmos OU, mild mucoid discharge from nose, clean ears. Moderate tartar and gingivitis.
PLNs: Not enlarged.
H/L: NSR, NMA. Eupnic, quiet lung sounds.
Abd: Soft, no pain on palpation, no masses palpated
M/S/I: Amb x4 but severe muscle wasting. Fur heavily matted – many mats the size of golf balls. All nails very long. 4th nail on front right foot was curled around and slightly embedded into paw.
Shaved most mats off during intake exam.
Offered wet food and pt ate eagerly.
1. Severely dehydrated
2. Severely emaciated and muscle-wasted – R/O recent poor access to food compounding undering medical issue such as hyperthyroidism
3. Matted and long nails – shaved and nails trimmed
4. Dental disease
5. URI signs – mucoid ocular and nasal discharge
Short-term prognosis: Fair to good if diagnosis can be identified and appropriate treatment can be started
Current surgery status: Permanent waiver due to being geriatric and likely underlying disease
1. Shaved and trimmed nails during exam – there were some small nicks in the skin on the right flank, which were cleaned and repaired with tissue glue
2. LRS 75 ml SQ BID x5 days
3. CBC/chemistry/thyroid testing
4. Doxycycline 50 mg/ml 0.3 ml PO SID x10 days
5. Will likely need long-term nutritional support +/- medication to treat likely underlying medical problem
S: eating well, IVF were turned off overnight (possibly dt line occlusion downstream)
EENT: severe dental tartar & calculi; OU enopthlamos, lenticular sclerosis and iris atrophy,
Int: dry flaky skin, matted hair clipped yesterday
CV: NMA, pulses no palpable, pale pink mm
Resp: clear lungs, eupnic, mild nasal discharge
MS: BCS 1/4, minimally ambulatory, walks and stands on metatarsals
Neuro: mentation QAR to BAR, hindlimb ataxia, CPs wnl, PLRs WNL
systemic disease (Dm vs CKD vs hyperthyroidism vs other)

pull IVC tomorrow
Pinky has not had a behavior evaluation but she is medical green (tolerates all handling).
