Subjective: Kitten is BARH.
Observed Behavior – At back of cage, allows to be picked up, but does not solicit attention. A little shy or timid, but warms up quickly. Little restraint required for exam. Allows all body parts to be touched.
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
T = NA P = wnl R = wnl BCS 4-5/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: healthy deciduous dentition
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: wnl; both testicles palpable scrotally
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Rectal: not performed
Assessment: Apparently healthy
Plan: Acceptable candidate for adoption or rescue. Okay to schedule neuter.
SURGERY: Okay for surgery
