I came into the shelter as a owner surrender on 11-Oct-2017, with the surrender reason stated as person health – cancer.

[DVM Intake] DVM Intake Exam DVM : 1431 Estimated age: 15 years Microchip noted on Intake? no History : surrender, history of diabetes, no medication or medical history provided, unsure of which insulin or dosage was being used. Subjective: BAR, hydrated Observed Behavior – Friendly,allows exam and treatment with minimal restraint. Evidence of Cruelty seen -no Evidence of Trauma seen -no Objective P = 120 R = wnl BCS: 5/9 EENT: clear OU, ears clean, no nasal or ocular discharge noted Oral Exam: mild staining, otherwise nsf. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male castrated. MSI: Ambulatory x 4, skin free of parasites, no masses noted, scruffy/unkempt hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: HX of diabetes Prognosis: fair to good Plan: Blood glucose on intake: 529 g/dl given 2 units of Humulin-N SQ @ 6:45 pm repeat BG’s, tx with insulin recommend BG curve, fructosamine levels continue to monitor at ACC SURGERY: already altered

KNOWN HISTORY: KNOWN HISTORY: Lived Indoors Previously lived with: Adults and children Behavior toward strangers: hides and for a few days Behavior toward children: Relaxed and aloof Behavior toward cats: Affectionate Behavior toward dogs: Unknown Bite or Scratch history: No Litter box training: Yes Energy level/descriptors: Shy, Affectionate, and quiet. Other notes: He isn’t bothered by being bath and enjoys being held. Phoenix will walk calmly in to the carrier. MEDICAL BEHAVIOR: Friendly,allows exam and treatment with minimal restraint. Cage Condition: Cage is slightly re-arranged Reaction to assessor: Phoenix was relaxed and lying down at the front of the kennel. Reaction when softly spoken to: Phoenix leans forward to sniff then gently rubs his cheeks against the kennel door. Reaction to cage door opening: Phoenix is calm and relaxed. Reaction to touch: Phoenix leans in for rubs then gently nudges the assessor’s hand and gives head butts. He rises up, nuzzles against the assessor and allows petting all over his body. Reaction to being picked up: Phoenix was a bit tense when picked up, but her remained calm and allows all handling. ACTIVITY LEVEL: Laid back VOCAL: Quiet CHARACTER TYPE: Sweet Easy going POTENTIAL CHALLENGES: Inappropriate elimination Potential challenges comments: Phoenix’s previous owner reports that he was eliminating (urinating) outside the litter box, but we are unaware of what, if any, modification steps were taken to try and address this behavior. Please note as well that Phoenix has been diagnosed with and is being treated for Diabetes in the care center, which may be influencing the urination behavior. We cannot be sure whether this behavior will continue in a home environment after treatment for the medical condition has been completed, but we recommend that potential adopters be comfortable with management and modification techniques for elimination outside the litter box should this behavior continue in a home environment. BEHAVIOR DETERMINATION: Beginner Behavior Asilomar H – Healthy RECOMMENDATIONS: None BEHAVIOR SUMMARY: Phoenix interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.
