Observed Behavior – friendly/vocal, allowed PE
Evidence of Cruelty seen – no
Evidence of Trauma seen – no

EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: nsf
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: female
MSI: Ambulatory x 4, flea dirt, otherwise healthy hair coat, no masses palp.
CNS: mentation appropriate – no signs of neurologic abnormalities
Rectal: not performed
Assessment – apparently healthy
Plan – fleas treated, none at this time
Prognosis: good
SURGERY: Okay for surgery
Profile:
Basic Information:
Phoebe is a 4 month old female DSH. She was bred in her previous owner’s home. She was surrendered because her previous owner is asthmatic and allergic. She has not been to the vet but has no known injuries or health concerns.
Socialization:
Around strangers, Phoebe is friendly and outgoing and gentle at play. She lived with her 4 month old sister and with her she was relaxed and affectionate.
Behavior:
Phoebe has never bitten a person or animal. In car rides she meows. Phoebe struggles if her coat is brushed. She isn’t bothered if placed in carrier or disturbed while sleeping. She enjoys being picked up and held.
For a New Family to Know:
Phoebe is described as friendly, affectionate, playful, confident and talkative with a high activity level. The two things her previous owner liked most about her were; she’s sweet and curious. When her previous owner was home she would tend to follow her around. Her favorite toys are balls and empty paper rolls. She was kept indoors only and at night she slept on her previous owner’s bed. She ate dry Purina. She is litter box trained and used an uncovered litter box and clumping litter. She likes to scratch on a vertical scratching post made of carpet and leather furniture as well.
Behavior during intake:
Phoebe allowed me to pet her and collar her but had a tense body. She head butted and purred and was relaxed when pet.
