*** SAFE 03/28/17 *** Pharah was brought into the care center after his owner fell ill and could no longer take care of him and his other 8 littermates. He now needs a follow up vet check, some good food and a lot of TLC.

I came in the shelter as a STRAY on 03/24/2017 from NY 10037, owner surrender reason stated was STRAY. I came in with Group/Litter #K17-092012.

KNOWN HISTORY: Pharah was brought in as a stray, so we cannot speak to his behavior in his previous home. He was receptive to all handling during intake and did not display any signs of aggression. MEDICAL BEHAVIOR: VERY friendly, purring, wants to be pet EVALUATION: Cage Condition: Cage is slightly re-arranged Reaction to assessor: Pharah looks at the assessor, is soft and relaxed. Reaction when softly spoken to: Pharah meow and comes to the front of the cage, sniffs the assessor’s fingers between the bars. Reaction to cage door opening: Pharah remains calm and at the front. Reaction to touch: Pharah leans and arches into the stroke, and purrs. Reaction to being picked up: Pharah sits calmly in the assessor’s arms and looks around. ACTIVITY LEVEL: Mellow VOCAL: Quiet CHARACTER TYPE: Social, Sweet, Affectionate BEHAVIOR SUMMARY: Beginner Pharah interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.

03/26/17 08:29 S: excellent appetite, IVC patent (flushed with heparin today), IFV at 6ml/hr, growling and trying to bite while restrained, but likes to be pet when in kennel O: persistent skin turgor EENT: no discharge AU/OU/nares Oral: no ulcerations on tongue or hard palate, gingival rescession and tartar on canine, PM & M not visualized dt behavior Int: no lesions, generalized dry unkempt haircoat (improved since intake) Lnn: WNL CV: NMA, s&s pulses, pale pink moist mm Resp: clear lungs, eupnic Abd: SNP UG: neutered male MS: 4x ambulatory, BCS 2/9 Neuro: BAR, no obvious neuro deficits, complete neuro exam not performed A: CKD emaciation skin tugor r/o loss of sq fat vs dehydration vs other dental disease P: cwct replace IVC vs switch to sqf 3/27 cbc/chem px: fair, pre-renal component likely continuted to degree of azotemia seen at intake, and true extent of renal disease can be better assessed when rehydration achieved 3/25/17 S/O: BAR, IV line running, offered cat ate well ,VERY friendly, purring, wants to be pet, urine in box M/S: weak, but amb x4, BCS 2/9 EENT: light pink gums, mild tartar, OU moderate mucopurulent d/c with mild conjunctivitis. No petecchiations, mild dark aural d/c AU SKIN: improved hydrated- slightly slow skin tent ABN: SNP UROGEN: MI, both testicles descended LN: NSF NEURO: NSF A: emaciated- eating well dehydrated- improved dramatically OU: conjunctivitis, eye infection AU: r/o ear mites MI mild dental dz BW performed yesterday revealed HCT of 57%- r/o dehydration, stress leukogram, low platelets of 12 (r/o spurious, other), hyperglycemia of 251- r/o stress, BUN>130, CREAT 1.5, PHOS 11.7: r/o severe dehydration, underlying renal dz, increased GLOB and TP: r/o secondary to dehydration,mild elevation in NA: r/o secondary to dehydration, other; mild elevation in ALK PHOS and GGT of 9: r/o secondary to HL, underlying liver dz, other. T4 normal P: add on: terramycin OU BID x10 con’t IV fluids for the day changed age to 6yrs, as p teeth consistent with adult aged cat made NH aware of p although p has significant abN to BW and exam, given that p seems to be responding and has good app is positive. Prog: with appropriate care, testing could be good. Would benefit from continued hospitalization until stronger/adds on weight and blood values improve. If they do not, consider AUS. Also rec UA, fecal OPG, repeat cbc/chem in 2 days to monitor values
