I came in the shelter as a STRAY on 08/15/2017 from NY 10314, owner surrender reason stated was STRAY. I came in with Group/Litter #K17-107965.

DVM Intake Exam Estimated age: 9 weeks History : stray Subjective: Observed Behavior – tense, tolerated exam when restrained Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective BCS 5/9 EENT: OD- conjunctival hyperemia and green mucoid discharge, OS- clear, AU- mild brown waxy debris, clear mucoid nasal discharge Oral Exam: deciduous teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI, 2 descended testicles MSI: Ambulatory x 4, no masses noted, large amount of flea dirt CNS: mentation appropriate – no signs of neurologic abnormalities Assessment nasal discharge- R/O URI vs other fleas- treated with capstar and frontline 8/16/17 Plan start doxycycline 12 mg PO SID x 10 days start erythromycin OU BID x 7 days Prognosis: good SURGERY: Temporary waiver due to URI

KNOWN HISTORY: Phantasm was brought in as a stray, so we cannot speak to his behavior in his previous home. MEDICAL BEHAVIOR: Observed Behavior – tense, tolerated exam when restrained ENRICHMENT NOTES: 08/17/17 Immediately starts to hiss upon approach and shifts away in an attempt to hide. Attempts to escape as the cage door opens and dodges petting. Really fearful of interaction. EVALUATION: Cage Condition: Cage is re-arranged Reaction to assessor: Phantasm is lying in the back of his kennel with litter mates. He makes eye contact with assessor and continues to lay motionless. Reaction when softly spoken to: Phantasm makes eye contact and continues to lay in his kennel. After a short time he closes his eyes. Reaction to cage door opening: Phantasm stays in the back of his kennel. He remains motionless and apparently calm. Reaction to touch: Phantasm allows petting all over. His body is slightly tense but he leans in softly for chin rubs and closes his eyes while being pet. Reaction to being picked up: Phantasm tolerates being picked up. His body remains tense but allows all handling. After a short time Phantasm goes back into his kennel to lay down. ACTIVITY LEVEL: Laid back, Mellow VOCAL: Quiet CHARACTER TYPE: Calm, Sweet, Shy BEHAVIOR SUMMARY: Average Phantasm interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. He may still need time to warm up to his new home and family, but this cat is showing behavior appropriate for new or experienced cat parents.
