History : owner surrender Subjective: BAR Observed Behavior – a little skiddish but easy to handle and examine Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T =not taken P = WNL R = eupnic BCS 6/9 EENT: Eyes clear, no nasal discharge noted AU-copious amount of dark brown/black discharge; very pruritic Oral Exam: mm pk, moist; CRT <2 sec; moderate tartar/gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: soft, nonpainful; slightly doughy with no palpable abnormalities U/G: neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, hair coat slightly unthrifty with mild flea debris noted CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 1. Ear mites-r/o otitis externa 2.. mild dental disease 3. flea debris Plan clean ears, tresaderm AU SID x 10 days rec’d dental rec’d monthly flea treatment rec’d dietary management to prevent obesity Prognosis: good to excellent neutered
