Medical Summary DVM Intake Exam Estimated age: 3 months Microchip noted on Intake? No History : No previous hx. Brought in 9/18/17 with an open degloving wound involving the majority of the tail Subjective: BAR, friendly, sweet, good for medical handling, purring Evidence of Cruelty seen – No Evidence of Trauma seen – Yes, the tail P = 200 R = 44 BCS = 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted. Oral Exam: No oral/lingual ulcers. Clean dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Soft and non painful, no masses palpated U/G: intact male, 2 testicles decended MSI: Ambulatory x 4, no ectoparasites seen, no masses noted, healthy hair coat. Degloving injury involving the maority of the tail. Dorsal proximal 2/3 of tail has fresh open wound, minimal bleeding, mild mucopurulent discharge. Caudal 1/3 of tail has a thick black scab, with stuck on debris. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 3m MI DSH Degloving tail injury Plan Proximal tail amputation scheduled for tomorrow 9/20/17 Neuter scheduled for tomorrow 9/20/17 Clean and bandage tail Simbadol 0.24mg/kg SQ SID x 4d Clavamox 13.75mg/kg PO BID x 7d Prognosis: Fair SURGERY: Okay for surgery
