Behavior Assessment Date: 10/4/2017 Retest Date: Retest Reason: Next Test Date: KNOWN HISTORY: Peluche was brought in without information on his behavior history in a home environment. ENRICHMENT NOTES: 10/04/17 Lying in litterbox with a low body and tilted ears. Tail is wrapped around body. He lifts his head and makes eye contact when spoken to. Tolerates petting along his head and body while remaining very still, ears flattened. Uncomfortable. He sniffs the treats I place down but won’t eat them during our interaction. Needs more time to adjust. Cage Condition: Cage is neat Reaction to assessor: Peluche was lying down in his litter box and had a low body posture. Reaction when softly spoken to: Peluche averts his gaze and his ears tilt slightly sideways. Reaction to cage door opening: Peluche remains motionless. Reaction to touch: Peluche sniffs the assessor’s hand and accepts petting. He warms up and arches his body, and then leans in for cheek rubs. He slowly gets up and rubs his head against the assessor. Reaction to being picked up: Peluche is a bit tense during pick up, but he remains calm when held. Started on clavamox and TAB for conjunctivitis Subjective: Observed Behavior – QAR, ~5% dehydrated Objective: P = wnl R = wnl BCS 5/9 EENT: OU clear no d/c, OS blepharospastic, ears clean, no nasal discharge noted Oral Exam: Clean adult dentition with gingivitis, no oral lesions PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MN MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, small lacerationon the top of the nose CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: Lethargy Diarrhea Hemorrhage at the Right conjuctiva r/o trauma vs infection vs other r/o trauma vs laceration-resolved Plan/Treatment/Recommendations: d/c eye meds Metronidazole 15mg/kg PO BID x5d CBC-monocytosis 0.72 (0.05-0.67) Chem-mild hypocholesterol 50 (65-225), mild hyperglobulinemia 5.5 (2.8-5.1) LRS 100ml SQ once Cerenia 1mg/kg SQ Fecal-2+ coccidia Recheck tomorrow Fluorescein stain OU-negative ponazuril 30mg/kg PO once Prognosis: Good
Re-exam overall condition History: Owner surrender 9/30. Started on clavamox and TAB for conjunctivitis. 10.7 was having diarrhea-coccidia +. Started on metronidazole and given ponazuril as well as LRS and cerenia for dehydration and decreased appetite. 10/7: CBC-monocytosis 0.72 (0.05-0.67) Chem-mild hypocholesterol 50 (65-225), mild hyperglobulinemia 5.5 (2.8-5.1) Fecal-2+ coccidia Subjective: Observed Behavior – QARH. Blepharospastic but no d/c. No c/s/v/d. Overall looks dull/not thrifty. Eating well. Objective: EENT: OU clear no d/c, OU blepharospastic, ears clean, no nasal discharge noted H/L: eupneic, no c/s Assessment: Lethargy Diarrhea Hemorrhage at the Right conjuctiva r/o trauma vs infection vs other r/o trauma vs laceration-resolved Dehydrated-resolved Decreased appetite-resolved Plan/Treatment/Recommendations: CTM as needed while at BACC Recheck fecal 10/16 Continue Metronidazole 15mg/kg PO BID Prognosis: Good
URI diagnosed on rounds S/O: QAR. Shyed away from petting, ears went back slightly when trying to examine, tried to get away. Decreased appetite. Very mild dehydration EENT: Eyes clear, mild to moderate serous nasal discharge, hypersalivating HL: Sneeze marks in cage A: URI, hypersalivating, mild dehydration P: Rec doxycycline 0.9ml PO q24 x 10 days, LRS 100ml SQ and cerenia 0.4ml SQ once tonight. Continue to monitor while at BACC. Excellent prognosis
PE:wnl Exam & surgery performed by Dr.1431 Feline neuter Anesthesia – .15mL Telazol im induction. Sx. – Routine scrotal castration. No sutures placed, incisions left open to drain. Green linear tattoo placed on ventral abdomen. 0.55mL simbadol sq injectable and 0.41mL onsior sq for pain management.
