Medical Summary DVM Intake Exam Estimated age: 4-8 years old based on overall appearance, muscle wasting and dentition Microchip noted on Intake? Scanned neg – MC placed during LVT intake History : Stray, found outside, appeared to be ill Subjective: Calm, allows all handling. Moves slowly, underweight. Objective QAR, estimated 8% dehydrated based on decreased skin turgor, BCS 3/9, MMs pale pink and moist EENT: Eyes clear, mild crusty dried nasal discharge at corners of nose – no active discharge. AU moderate erythema and black granular discharge. Oral Exam: Mild tartar, gingivitis. Fractured right maxillary canine tooth at the neck – moderate gingivitis here. PLN: No significant enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact, testicles S/S MSI: Emaciated, dehydrated, mild-moderate muscle wasting generally but especially in pelvis and hind limbs. Slight ataxia and occasional falling to one side – not consistent which side it is. No fractures, swellings, wounds noted on either hind limb. Possible slight plantigrade stance. Dirty fur coat. CNS: mentation appropriate Rectal: Normal externally Assessment: 1. Emaciation, dehydration, muscle wasting – R/O renal dz, diabetes, hyperthyroidism, other 2. Falling to one side, plantigrade stance – R/O weakness vs. true neurological deficit 3. Fractured tooth, gingivitis at this site – R/O root infection and pain Plan: 1. Run CBC/chemistry 2. LRS 200 ml SQ SID x5 days 3. Simbadol 0.5 ml SQ SID x10 days – recommend dental cleaning with rads and extraction of this tooth ASAP Prognosis: Fair SURGERY: Temporary waiver pending CBC/chemistry results 1088
