I came in the shelter as a OWNER SUR on 07/21/2017 from NY 11236, owner surrender reason stated was PETS CONFL.

PE:Barh- fiv- fleas allergic dermatitis Exam & surgery performed by Dr. Bruno Feline neuter Anesthesia – 0.15 mL Telazol induction. Sx. – Routine scrotal castration. No sutures placed, incisions left open to drain. Green linear tattoo placed on ventral abdomen. 0.63mL Simbadol pre op injectable and 0.52 mL onsior post op for pain management. Start to clavamox drop 1.1 ml po bid x 7 days

07/21/17 11:08 Owner stated that the cat has lived at her workplace since his birth, as his Mother was in the warehouse before she started. The cat allows for handling, he let the owner pick him up and place him in the carrier without a struggle. The owner stated that he allows for pets and rubs his head against her leg, especially when he knows she is about to feed him. He rode quietly in the car and sat well in the carrier while in the shelter. When being collared he struggled to run away but did not hiss, swat or attempt to bite counselors.

KNOWN HISTORY: Papa was brought in with limited information on his behavioral tendencies. He lived in a warehouse and was reported to be friendly towards the workers there; he approaches them and allows them to pet him. He allows handling and was placed inside a carrier without issue. He likes to rub against a person’s legs, especially when he knows he’s about to be fed. He also allowed all handling during the intake process. MEDICAL BEHAVIOR: Observed Behavior – Does not like to be handled, but allows to be scruffed. EVALUATION: Cage Condition: Cage is slightly re-arranged Reaction to assessor: Papa was lying down by the front and was calm, relaxed, and friendly. Reaction when softly spoken to: Papa looks at the assessor with soft eyes and rolls over to display his belly. Reaction to cage door opening: Papa retreats to the back and curls up. Reaction to touch: Papa approaches the front again and head-butts. He rubs his entire body on the assessor and begins to purr, but also attempts to leap out of his kennel to explore the room. Reaction to being picked up: Papa struggles to be held, but tolerates handling. After a moment, he leaps down and attempts to explore again. ACTIVITY LEVEL: Moderate VOCAL: Quiet CHARACTER TYPE: Social, Affectionate, Curious BEHAVIOR SUMMARY: Average Papa interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. No known history of behavioral problems. This cat is showing behavior appropriate for new or experienced cat parents.

Subjective: Intact male having alopecia, flea allergies dermatitis, eosinophilinc plaque, and FIV Observed Behavior – Do not like to be handled, Allow to be scruffed. Evidence of Cruelty seen – None Evidence of Trauma seen – None Objective P = 160 bpm BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Could not examined well. Indolent ulcer PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, skin ( fleas) of parasites, no masses noted, generalized alopecia. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment FIV Flea allergies alopecia Indolent ulcer Plan Revolution applied Steroid should not be used in this case ebecause animal has FIV. Rescue group or trained foster. Prognosis: fair. SURGERY: Okay for surgery
