Subjective:
Observed Behavior – BARH. Allows all handling, eating with excellent appetite
Evidence of Cruelty seen – No
Evidence of Trauma seen – No
Objective
P = WNL R = WNL BCS 3/9
EENT: Eyes clear, moderate conjunctiviits with mild mucoid ocular discharge, moderate mucoid nasal discharge, ears clean
Oral Exam: All deciduous teeth fully erupted, pink mm, no oral ulcers
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Male
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: Underweight, URI
Plan: Rec doxycycline 0.2ml PO q24 x 10 days and erythromycin OU q12 x 10 days
Prognosis: Excellent
SURGERY: Temporary waiver due to URI/underweight
