History : Stray kittens found in the yards.
Subjective: URI,ringworm, conjutivitis orphan kitten
Observed Behavior – BAR
Evidence of Cruelty seen – None
Evidence of Trauma seen – None
Objective
T =Not taken P = 240 bpm R = Not taken BCS 4/9
EENT: bilateral conjuctivitis, ears clean, no nasal discharge noted
Oral Exam: NAF
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Intact male
MSI: Ambulatory x 4, skin free of parasites, no masses noted, patches of alopecia around the head.
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment
Ringworm, conjuctivitis
Plan
Pyrantel, frontline, FVRCP vaccine, ponazuril,
Lyme dip.
DTM test- pending
Woodslamp- positive
FELV/FIV- Negative
erythromycin eye ointment BID OU for 10 .
Prognosis: Fair
Temporary waiver due to underweight, ringworm and conjuctivitis
