Medical Summary DVM Intake Exam Estimated age: 4 mos Microchip noted on Intake? neg History : o surrender two kittens from same household – Optimus A1115174 and Chromio A1115175 Subjective: BARH. Nasal discharge – URI. Ate canned but not too interested in dry. Observed Behavior – able to do exam but nervous and may run. Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective T = P = R = BCS= 5/9 EENT: Eyes mild chemosis/ conj erythema and slight MPOD, ears clean, SND with slight mucus. Oral Exam: NSF – no ulcers. PLN: No enlargements noted H/L: NSR, NMA, MM pk, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated. U/G: Female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: n Assessment Intact female. URI Plan: Doxy liq 0.4 cc po sid x 10 d. Monitor appetite. Prognosis: Excellent SURGERY: Temporary waiver for spay due to URI.
