I came in the shelter as a STRAY on 08/27/2016 from NY 11434, owner surrender reason stated was STRAY.

8/27/16 Microchip scan neg. O: BARH. mm=pink, moist, CRT<2 s. EENT: Mucoid discharge OU. Mucoid nasal discharge, congestion, sneezing. Abd: Palpation WNL. UG: Intact male. A: URI. P: FVRCP. Panacur 0.32 ml PO, Ponazuril 0.16 ml PO. Cleaned OU and applied Terramycin. Rx Terramycin oint. Apply OU BID x 10 days. Rx Doxycycline suspension. Give 0.15 ml PO q 24 hrs x 10 days. Prognosis excellent.
