Medical Summary DVM Intake Exam Estimated age: 6 weeks Microchip noted on Intake? no History : found stray, no medical history Subjective: QAR, euhydrated Observed Behavior – quiet but sweet, allows all handling Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 5/9 EENT: Moderate serous ocular and nasal discharge. Severe dark brown crusty discharge with pruritus AU. Oral Exam: deciduous teeth present, no oral ulcers PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female MSI: Ambulatory x 4, skin free of parasites, no masses noted, mild hair matting and discharge on the left forelimb consistent with ocular/nasal discharge CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: URI, ear mite infestation, underage/underweight Plan: Start erythromycin ointment OU BID x 10 days and doxycycline 0.2 mL PO SID x 10 days, Gave Revolution at intake for ear mites. Recheck in 10 days or earlier if worsening. Prognosis: good SURGERY: Temporary waiver due to underage/URI
