I came in the shelter as a STRAY on 08/25/2017 from NY 11223, owner surrender reason stated was STRAY.

Good appetite P = wnl R = wnl BCS 2/9 EENT: moderate purulent nasal d/c bilaterally, moderate mucopurulent ocular d/c ou with conjunctivitis, chemosis, and blepharospasm Oral Exam: kitten dentition, no oral ulcers noted PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic but audible upper respiratory congestion ABD: Non painful, no masses palpated, distended U/G: MI MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt haircoat CNS: mentation appropriate – no signs of neurologic abnormalities A: Underweight URI Conjunctivitis Dehydration P: Continue with current treatment (doxycycline 10mg/kg PO SID and erythromycin OU BID) and monitoring plan.
