Subjective / Observed Behavior – BAR, allows all handling.
Evidence of Cruelty seen – none
Evidence of Trauma seen – yes, 1cm wound with myiasis along perineal region

BCS 3/9
EENT: Eyes crusted shut with mucoid discharge, ears have mild otic debris, mucoid nasal discharge
Oral Exam: normal kitten dentition
PLN: No enlargements noted
H/L: No murmur ausculted; CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: male; unable to palpate testicles. wound in region of testicles
MSI: Ambulatory x 4, heavy flea dirt infestation; dead fleas present; no masses noted, hair coat is dirty. 1cm x1cm wound ventral to anus and dorsal to penis with dead maggots
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: mild erythema of the perineum
Assessment
URI — moderate; viral and bacterial
myiasis — under tx
underweight
flea infestation — under tx
mild otis debris — r/o otitis externa
Prognosis:
fair
Plan:
clean and flush wound with dilute chlorhexidine
hydrotherapy on wound daily
Apply SSD to wound daily
doxycycline 50mg/ ml — 0.1ml PO q24h x 7 days
clean eyes and nose as needed daily
vitamin b12 injection 0.1ml one time
fortiflora q24h
nebulization q24h x 5 days
monitor appetite and overall well being
SURGERY:
Temporary waiver due to weight
