SAFE 4/17/18 **BLIND** WATCH VIDEO – NINO is about 3 yrs old and was found wandering around an apartment building. He seems to be blind and is not feeling well. Nino has diarrhea, is lethargic, dehydrated and emaciated and has a heart murmur and needs a full medical work up to determine what is wrong. He needs help asap. Please give this handsome boy a chance by offering to foster or adopt.

Subjective / Observed Behavior – QAR, apparently blind; allows all handling but gently resists.

EENT: Eyes clear; pupils dilated, bilaterally. minimal menace response OS; absent OD. ears clean, no nasal or ocular discharge noted
Oral Exam: dc 1/5; pd 1/5.
PLN: No enlargements noted
H/L: NSR, NMA, CRT 2; lt pink; mm dry, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: intact male.
MSI: Ambulatory x 4, skin free of parasites, prolonged skin tent; no masses noted, hair coat dull
CNS: Mentation appropriate; partial to complete blindness. absent menace OD; minimal menace OS
Rectal: fecal staining present, otherwise normal.

CBC
chemistry — postponed until 4/14. unable to obtain sample.

IV catheter placement
LRS fluids IV 6ml/ hr x 24 hours
metronidazole 100mg/ml —
praziquantel 0.2ml IM one time
sedated abdominal rads — 0.03ml torb / midaz
monitor hydration and for iatrogenic heart murmur
entice to eat.
SURGERY:
Temporary waiver due to illness.
HCT 30 (WNL, but low normal)
Severe neutrophilia 43% – r/o infection
