Medical Summary DVM Intake Exam Estimated age: 1-2 years old Microchip noted on Intake? none History : Stray, in a boiler room in the Bronx Subjective: QAR, moderately prolonged skin turgor Observed Behavior – Very nervous and scared, flighty but no aggression Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = P = 220 R = eup BCS 2-3/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: clean adult dentiiton, 204 tip fx with pulp exposure PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: F, assumed intact MSI: Ambulatory x 4, , no masses noted, patchy alopecia and live fleas, moderate generalized muscle atrophy CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: NP Assessment 1-2 year old F (assumed) 1. Fleas with alopecia 2. Dehydration 3. Poor body condition Plan -100 ml SQ LRS today -Actyvil, pyrantel, MC, retro-negative -Send to surgery in 1 week if gaining weight and doing well Prognosis: Good SURGERY: Temp waiver for dehydration and poor condition
