***SAFE 07/13/17*** SWEET BEGINNER KITTY NICK IS SOCIAL AND AFFECTIONATE AND NEEDS SOMEONE TO GIVE HIM A HOME AND TLC!! NICK was found walking with some right hind lameness, and has injured paw pads. 07/10/2017 Exam Type RE-EXAM – Medical Rating is 4 C – SEVERE CONDITIONS , Behavior Rating is NONE, Weight 6.9 LBS.
7/10/17 Hx: URI on DVM Intake Exam 7/8/17 – 8% dehydration, ddz, emaciated, abrasions on paw pads. S) Friendly, head butts, seeks affection,easy to handle. O) New weight = 6.88, up from 6.7 BAR. Minimal dehydration -3%. MM pk. EENT: Eyes clr. SND. No c/s. Oral – no ulcers, mod- marked gingivitis with some bleeding of gums, rodents ulcers, some discomfort noted on palpation, mod tartar. MSI: BCS= 2/9, amb x 4. Paw pads – some pink patches but healed well, no pain/ swelling/ discharge. H/L good. Abd: SNP, NMP Eating canned well. Normal formed BM and urine in box. A) URI – improving. Emaciated but eating. Ddz. Dehydration improved – mild. P) Cont doxy. LRS 100 cc x 2 more days. Add buprenex oral 0.3 PO BID x 3 days – for dental pain. Prognosis: Good if continues to respond. Will need dental prophy – may help with eating. Recom foster/ rescue.

KNOWN HISTORY: Nick was brought in as a stray, so we cannot speak to his behavior in his previous home. MEDICAL BEHAVIOR: Attention seeking, allows all handling. Shows interest in food but not eating ENRICHMENT NOTES: 07/09/17 Comes forward as I open the kennel door and head-butts my hand in greeting. He allows petting along his head and body while leaning in, remaining in front of his kennel. Appreciates attention despite injury. Doing well today! 07/10/17 At front of kennel when I approach. He leans into my hand when I touch him and allows petting along his head and body, tail raised. Very friendly. Did well today! Likes treats. 07/11/17 Social and friendly at the front of the cage. Gives head butts and appreciates petting/attention. He steps out the cage to explore. Stays close to my side, rubs his body against mine. Sweet and affectionate. Didn’t mind being picked up. ACTIVITY LEVEL: Mellow VOCAL: Quiet CHARACTER TYPE: Social, Affectionate BEHAVIOR SUMMARY: Beginner Nick interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. No known history of behavioral problems. This cat can go to a beginner home.

Finder reported was not eating and was walking gingerly, not trying to avoid cars Subjective: Observed Behavior – Q/BAR. ~8% dehydration. Attention seeking, allows all handling. Shows interest in food but not eating Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective P = WNL R = WNL BCS 2/9 EENT: Eyes clear, ears clean, mild mucoid nasal discharge noted Oral Exam: Moderate dental tartar/gingivitis, severe halitosis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt hair coat, mild right hind lameness, paw pads on all feet appear to have sloughed and are in healing state CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Mild URI, emaciated, dehydrated Plan: Rec doxycycline 0.7ml PO q24 x 10 days, LRS 100ml SQ q24 x 3 days Prognosis: Good if responds to supportive care appropriately SURGERY: Temporary waiver due to being underweight/URI
