DVM Intake Exam Estimated age: ~3-4yrs Microchip noted on Intake? no History : Subjective: BAR. ~5% dehydration Observed Behavior – Active, attention seeking, head butting, allows handling but struggles with restraint (less is more with this cat) Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = WNL R = WNL BCS 4/9 EENT: Eyes clear, moderate conjunctivitis OU, mucoid ocular discharge, ears clean, mild mucoid nasal discharge noted Oral Exam: Mild dental tartar, pink mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty full hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: URI, underweight, dehydration, conjunctivitis Plan: Rec doxycycline 0.8ml PO q24 x 10 days and erythromycin OU q12 x 10 days (first dose given of both), LRS 100ml SQ administered. Face cleaned. Adding nebulization q24 x 3 days. Prognosis: Good SURGERY: Temporary waiver due to URI

KNOWN HISTORY:: Neo was brought in as a stray, so we cannot speak to his behavior in his previous home.
MEDICAL BEHAVIOR:: 01/12/18 Active, attention seeking, head butting, allows handling but struggles with restraint (less is more with this cat)
Cage Condition:: Cage is recently cleaned
Reaction to assessor:: Neo chirps and comes forward immediately when the assessor approaches the kennel.
Reaction when softly spoken to:: Neo purrs and rubs her cheeks against the kennel door.
Reaction to cage door opening:: Neo remains calm and relaxed.
Reaction to touch:: Neo purrs, leans in and gives the assessor head butts when pet. He’s social, affectionate and allows petting all over his body.
Reaction to being picked up:: Neo was calm, relaxed and allows all handling.
