04/26/17 12:47 Upon intake Neil Young was calm and relaxed at first. When outside the carrier he started to hiss, sniff around and hide. When I opened a can of wet food he came out and starting eating. He scanned negative for a microchip and allowed all handling.

ACTIVITY LEVEL: Laid back VOCAL: Quiet CHARACTER TYPE: curious, skittish EVALUATION: Cage Condition: No change Reaction to assessor: Neil looks neutral, lying down on his cage bedding. Reaction to cage door opening: Neil remains immobile in place, alert with eyes wandering. Reaction to touch: Neil accepts the touch, but stiffens up and shows no interest. BEHAVIOR SUMMARY: Neil may be a little more independent, and may need time to warm up to his new home. He may have some behavioral issues that will need to be addressed in the home. We recommend that this cat go to a home with experienced cat parents.

History : brought in as a stray Subjective: BAR Observed Behavior – interactive and allows petting and gentle handling initially but became more fractious after a few minutes-unable to combo test or MC due to temperament; ate canned food well in carrier Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T = not taken P =WNL R =WNL BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, moist; CRT <2 sec; teeth look very white with minimal tartar; tip fracture of upper L canine with no obvious root exposure PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact with 2 normal descended testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 1. Apparently healthy young adult Plan no treatment needed Prognosis: excellent SURGERY: Okay for surgery

05/01/17 09:07 Hx: hematuria noted day after intake (4/27), sedated X rays on 4/28 showed large bladder and large amount of feces, no pelvic bone abnormalities, bladder was easily expressed with no hematuria on 4/28, but continued straining noted in box, small amount of dry feces produced, was started on Miralax and Enulose & LRS. Due to behavioral aggression, only sedated exams have been done with visual exams in between. Was anesthetized yesterday 4/30 for neuter and enema, soft feces removed from colon. Nofurther straining noted/ S/O: eating very well, urine but no feces in box this morning (both urine and feces noted overnight) BAR no discharge OU or nares 4x ambulatory no straining noted this morning A: constipation P: cw enulose and miralax monitor BM discont Simbadol and Prazocin px good
