Medical Summary DVM Intake Exam Findings History Owner surrender, no health concerns reported Subjective Quiet, slightly tense, doesn’t move much, allows handling Objective BCS 8/9, MMs pink and moist EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Moderate gingivitis and tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, reported spayed MSI: Polydactyl – 1-2 extra toes on all four feet. Right front foot – extra toenail is curled around and embedded in pad. Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Trimmed nails – embedded nail came out easily and left a small wound, no swelling, mild bleeding Assessment: 1. Obese 2. Polydactyl, one nail embedded –> small wound 3. Dental disease Plan: 1. Recommend weight loss of 3-6 lbs 2. Yesterday’s News, recheck RFL paw pad in 2 days 3. Recommend dental cleaning 1088
