SAFE 5/9/18 NATHANIEL was dumped by his owner. He is extremely upset. Nathaniel has a history of hyperesthesia syndrome and is on medication for that. He also needs some follow up care. Please help this kitty today.

History: Surrendered by o. O reports P has hx of hyperesthesia syndrome diagnosed April 2018. P is on fluoxetine 4mg/ml — 1ml q24h. Disease is characterized by self trauma and aggression towards the owner; these are new behaviors for P.
Subjective / Observed Behavior – BAR; did not allow any handling. Hissing, swatting with claws out. Growling in kennel even when not approached. Netted and sedated for PE.
Evidence of Cruelty seen – none
Evidence of Trauma seen – none
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: dc 1/5; pd 1/5.
PLN: No enlargements noted
H/L: No murmur ausculted; CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: intact male. testicles smooth and symmetrical.
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate; unable to perform neuro exam due to behavior. Hyperesthesia along the caudal end while under DKT.
Rectal: grossly normal.

0.2mls Dexdomitor 0.5mg/ml, 0.2mls Butorphanol 10mg/ml IM / 0.2mls Ketamine — adequate sedation
Administered 0.2mls Antisedan 5mg/ml IM
gabapentin 0.6ml PO q12h x 3 days for ease into shelter and neurologic pain
continue on fluoxetine 20mg/5ml (4mg/ml) — give 1ml PO q24h
