Medical Summary DVM Intake Exam Estimated age: 3-4 years Microchip noted on Intake? no History : stray Subjective: QAR, 5-7% dehydrated Observed Behavior – quiet, alert, unsteady, easily handled and restrained Evidence of Cruelty seen – malnutrition Evidence of Trauma seen – no Objective T = 100.7 P = 120 R = 20 BCS: 2/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: negative oral exam PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated, very thin, firm stool in colon U/G: female intact, no spay scar noted MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate, seems alert, but very weak to ataxic, no CP deficits, no nystagmus or obvious cranial nerve deficits. staggering, weak gait. Assessment: Thin to cachectic, dehydrated, malnourished, vaguely neurologic/weak/ataxic: rule out secondary to malnourishment vs cerebellar hypoplasia vs infectious vs inflammatory vs neoplasia vs metabolic vs other. Ate A/D well and readily in room. Plan: vaccinate microchip flea tx deworm Panleuk test (parvo snap) : negative combo snap test: consider full bloodwork including TT4 recommend going to medical for observation/supportive care, 100 ml LRS sq today. Prognosis: guarded to poor SURGERY: Temporary waiver for OHE due to cachexia and weakness/ataxia
