Medical Summary DVM Intake Exam Estimated age: 2yrs Microchip noted on Intake? yes 981020019631816 History : Subjective: returned by owner because more active of a cat than o realized Observed Behavior – docile, allows all handling, no aggression Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective offered canned food- ate readily P = 180 R = 60 BCS 5/9 EENT: Eyes clear, ears clean– no d/c or abN noted in ear canals, no nasal discharge noted Oral Exam: very mild gingivitis, pink gums PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MN, grossly normal MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, some moderate fur thinning on caudal aspects of pinnae bilaterally- with sparse regrowth, skin beneath normal CNS: mentation appropriate – no signs of neurologic abnormalities Assessment fur thinning ear pinnae: r/o normal for p, reaction to topical, other very mild periodontal dz Plan ok for adoptions Woods lamp pinnal skin: negative monitor fur on pinnae for regrowth long term if p tolerates, can brush teeth with cat toothbrush and paste SID for oral health Prognosis: good SURGERY: n/a, already neutered

Objective BCS 5/9, MMs pink and moist EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Clean teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact, testicles S/S MSI: Ambulatory x 4, no masses noted, healthy hair coat. Many small scabs on dorsal neck. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 1.
