Medical Summary DVM Intake Exam Estimated age: 5yr old Microchip noted on Intake? negative on intake , placed MC upon LVT intake History : found as stray Subjective: vomited shrimp and undigested food overnight, drinking water prior to exam, no vomitus in cage Observed Behavior – friendly allows handling with no aggression, purring Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = not taken P = purring unable to obtain R = purring, but eupneic BCS 5/9 EENT: Eyes clear, ears clean, scant nasal discharge noted, sneezed once purulent tinged d/c Oral Exam: elongated L maxillary canine, mild gingivitis PLN: No enlargements noted H/L: NSR, no murmur asculted, CRT < 2, Lungs clear, eupneic, purring ABD: Non painful, no masses palpated, no fluid palpated, but enlarged, cannot palpate distinct embryos U/G: slightly developed nipples, no palpable MGTs, vulva grossly normal MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment V once overnight distended abdomen- r/o pregnant, masses, ascites, other L maxillary canine- likely diseased and should be extracted long term, mild periodontal dz FI Plan consider abdominal rads doxycyline 50mg/ml: 0.9ml PO SID x10days nebulize BID x3 days monitor for vomiting Prognosis: good SURGERY: Temporary waiver due to URI
