***SAFE 12/03/17 *** Lilac Point girl Moonstone needs a comfy retirement home @MACC. Poor Moonstone is a 16 yr old senior cat that was surrendered to the shelter because owner stated the cat has liver issues. Moonstone deserves to live out the rest of her golden years in a loving home.

I came into the shelter as a owner surrender on 27-Nov-2017, with the surrender reason stated as person circumstance- cannot afford to care for.

Moonstone is at risk for medical reasons. Moonstone is a geriatric lady with a number of underlying medical conditions, including renal failure, which cannot be adequately managed in the shelter. Behaviorally, Moonstone has been assessed as being suitable for placement in most homes.

DVM Intake Exam Estimated age: Reported 16 years old – exam is consistent with this Microchip noted on Intake? Scanned negative on intake History : Client reports that pt has liver problems but he doesn’t know what kind. Pt was eating a special diet but the client was unsure what diet it was. Urinating outside litterbox recently. Subjective: Mews at me, sits in one place unless I get her up. Observed Behavior – Allows all handling. Objective QAR-H, MMs pink and moist, BCS 7/9 with abdominal fat pad and mild muscle wasting. EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: Moderate tartar PLN: No enlargements noted H/L: Very soft heart sounds, NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, spay scar palpable MSI: Plantigrade stance, slightly weak or ataxic when standing. Skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally CBC/chemistry performed yesterday and available for review: Hct 40%, WBC 6.7 k/ul, plt 314 k/ul Slightly elevated creatinine (2.5, ref 0.8-2.4), BUN is high-normal at 35 (ref 16-36) Normal ALT, alk phos, GGT, bili Elevated cholesterol Elevated Na (168, ref 150-165) Assessment: 1. Reports of liver disease 2. Elevated creatinine with normal BUN – R/O CKD, with liver disease masking an elevated BUN, vs. CKD and muscle wasting contributing to a low BUN 3. Plantigrade stance – R/O weakness 4. Dental disease 5. Overweight Prognosis: Fair Plan: 1. Give 200 ml LRS SQ SID 2. Recommend liver-supportive supplements such as Denamarin and milk thistle (not currently in stock in shelter) 1088
Hx: hx of liver disease according to owner; mild azotemia noted on intake, liver values WNL; started on SQF S/O -BAR, resting quietly but then is very interactive when awoken; mews loudly for attention at front of cage -no appetite, appears nauseated when food is offered -no nasal discharge, soft sneezed once -OU: mild blepharospasm, mucoserous discharge OD -eupnic, grade III heart murmur noted -abdomen distended but soft and nonpainful with no palpable abnormalities -plantigrade stance A 1. Azotemia-r/o mild CKD with dehydration vs other 2. Heart murmur 3. Conjunctivitis 4. Plantigrade stance 5. Inappetance/nausea P -cerenia 0.5 ml SQ SID x 3 days -mirtazapine: 1/4 tab PO q3d -continue SQF -recommend rechecking chemistry in 3-5 days -no evidence of liver disease, it is possible that owner was mistaken and pet has mild renal disease instead

Upon intake, Moonstone was purring when counselor pet her. She allowed counselor to collar her and take her picture. she went inside carrier willingly.

Basic Information:: Moonstone is a 16 year old, female, domestic short haired cat. Previous owner got her from his previous partner who passed away. He has had her for a month and half but unfortunately has to surrender due to her having liver issues and being unable to afford her health care. He has not taken her to the vet since he has had her.
If yes, Please elaborate:: Moonstone has litter box accidents a few times a week. She is peeing outside of the litter box and it has been happening for the past 2 months. The previous owner has not taken her to the vet for this issue. They think it is because of her age.
Previously lived with:: 1 adult.
How is this cat around strangers?: Moonstone is friendly and outgoing with strangers. She does not play.
How is this cat around children?: Moonstone has not spent time around children.
How is this cat around other cats?: Moonstone has not spent time around cat.
How is this cat around dogs?: Moonstone has not spent time around dogs.
Behavior Notes: Moonstone has litter box accidents. She is relaxed during car rides. previous owner has never given her a bath or trimmed her nails. She enjoys being brushed and isn’t bothered when being picked up, put in carrier or disturbed while sleeping.
Bite history:: Moonstone has not bitten another animal or person.

Medical Notes: Previous owner stated that Moonstone has liver issues and is on a special diet for it but was unaware of the food she was eating.
For a New Family to Know: Moonstone is a friendly, affectionate, mellow, aloof cat. She has a low activity level and loves to sleep next to owner. She loves to give and get affection. She will stay in the same room as you when you are home and is indoors only. She eats her dry food 2 times a day. She is mostly litter box trained but has rare accidents a few times a week. She used an uncovered litter box with clumping litter. She used a cardboard scratching post.

MEDICAL BEHAVIOR:: 11/28 Observed Behavior – Allows all handling.
ENRICHMENT NOTES:: Resting in litter box, soft eyes and body. Sat up and started meowing on approach. Leaned slightly into pets, then came slowly forward to eat treats.
Cage Condition:: No change
Reaction to assessor:: Moonstone remains neutral, lying down on her cage bedding during the approach.
Reaction when softly spoken to:: Moonstone responds back with a meow.
Reaction to cage door opening:: Moonstone becomes alert with soft eyes, ears erect and forward.
Reaction to touch:: Moonstone accepts the touch and slowly leans into the assessor’s hand, appreciating the petting.
Reaction to being picked up:: She briefly allows the pickup then walks back into the cage.
