Medical Summary DVM Intake Exam Estimated age: Approx 2 months old Microchip noted on Intake? scan negative History : stray Subjective: Observed Behavior – friendly Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective BCS 4/9 EENT: OS- conjunctival hyperemia, green mucoid discharge, OD- clear, ears clean, no nasal discharge noted Oral Exam: deciduous teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment conjunctivitis OS underweight Plan start erythromycin OU BID x 7 days move to isolation Prognosis: good SURGERY: Temporary waiver due to conjunctivitis and underweight
