SAFE 5/11/18 BONDED WITH SIBLING APACHE – MITZY is a young kitty whose owner dumped at the shelter. Mitzy needs medical attention for some kind of fluid-filled mass in her abdomen. Please offer to foster so Mitzy can get the medical care she needs. Mitzy is very shy but is affectionate and friendly. She is attached to her sibling Apache (27018) and often tries to groom him.

H: On intake, 5/5, noted to have a flocculent swelling in the ventral abdominal SQ space.
CBC- Hct 35.1 (low-normal), otherwise unremarkable
Chemistry- BUN 27, Creat 1.4 (high end of normal)
T4- 2.4
– Aspirated 0.5ml of dark orange fluid from sac–> mild amount of rbcs seen, occasional epithelial cells, with occasional cluster of extracellular cocci (no intracellular bacteria noted, suspect stain precipitate)
Two view chest radiographs- unremarkable
Two view abdominal radiographs- Fluid-filled sac noted in the SQ space of the mid-abdominal region. No obvious bladder identified in normal location in abdomen (caudal ventral portion of the abdomen)
5/6 Repeat lateral abdominal radiograph- SQ swelling remains static in size, small bladder identified in proper place within abdomen
S: QAR to BAR; <5% dehydrated. Very nervous and tries to flee. Ate a small amount of food. Small amount of urine in the box. No BM. Spot of liquid (and maybe food?) in the cage- unknown if this is diarrhea or vomit. No cs.
Eyes: Grossly appropriate OU.
Ears: Unremarkable AU.
Nasal Cavity: No nasal discharge.
Oral Cavity: Mild dental tartar
PLN: WNL
Heart: No murmurs or arrhythmias.
Lungs: Eupneic, lungs clear. No crackles or wheezes bilaterally.
Abd: Soft, non-painful. No palpable masses. No organomegaly appreciated.
U/G: Normal external genitalia. No discharge.
Musculoskeletal: Ambulatory x 4 with no appreciable lameness.
BCS = 4/9
Integument: ~ 3-4cm in diameter, highly flocculent SQ swelling in mid to caudal ventral abdomen- appears dark in color, remains static in size from last exam.
Neuro: Appropriate mentation. Full neurologic exam not performed
Rectal: Not performed. Externally normal.
Assessment
1) Fluid filled sac in SQ space (caudal-mid ventral abdomen)- r/o old hematoma (from trauma, previous surgery) vs other mass- very unlikely this is bladder due to repeat radiograph
Prognosis: fair
Plan:
Added mirtazapine 3.75mg PO EOD for three doses, re-eval on 5/11 (only mild dehydration, concerned about giving SQ injections in light of SQ swelling)
CTM in medical- monitor urination, appetite closely as well as for decline in general condition
Alerted new hope- seek placement

MSI: Ambulatory x 4, skin free of parasites moderate flaky skin; ~ 3-4cm in diameter, SQ fluid filled highly deformable mass in mid to caudal ventral abdomen- appears dark in color
CNS: Mentation appropriate – no signs of neurologic abnormalities

Mitzy was lying in her litter box and had a low and tense body posture. Her ears tilt slightly sideways upon approach.

Mitzy avoids eye contact and looks around the room.
Reaction to cage door opening:
Mitzy’s eyes widen and she becomes alert.
Reaction to touch:
Mitzy tolerates petting along her head and body, but her ears tilt sideways and she turns her head to watch the assessor’s hand. She continues to focus on the assessor’s hand, and then shifts her weight.
