Medical Summary S/O: on ACC van BAR, food eaten, nice cat allowed hadnlin until attempted to examine ingrown nails understandably amb x4, BCS 3/5 SKIN: approx 2cm dermal mass that has opened and has purulent d/c and dried blood between shoulder blades, ingrown nails in FLs, unable to assess- painful EENT: pink gums, unable to fully assess oral cavity, mild purulent d/c from OU, otherwise eyes open and comfortable UROGEN: MC p tolerated exam until tried to touch nails then became fractious, otherwise was friendly unable to perform rest of exam A: mass infected: r/o benign vs. malignant ingrown FL nails OU: r/o mild bacterial infection P: buprenorphine 0.3mg/ml: 0.2ml at side of cheek BID x4 when p tolerates clip nails and soak in chlorhexidine solution, then use paper in litter covenia 80mg/ml: 0.5ml SQ once erythromycin OU BID x7 given Rabies RH Sq, unable to give FVRCP or pyrantel d/t temperment; no MC scanner available on van- did not perform As with any geriatric rec cbc/chem/t4/ua/fecal OPG as general health screen. Rec mass removal and biopsy to determine underlying cause and remove infected tissue. Once nails clipped and on treatments p will be much more comfortable.
