***SAFE 09/02/17*** MARVELLOUS MITCH IS A SWEET BEGINNER KITTY WHO DESERVES TO GET SOME MEDICAL CARE HER OWNER COULDN’T AFFORD!! 7 year old MITCH was brought into the shelter by her owner for pet health because owner could no longer afford to pay for further medical treatment. Mitch is a shy girl but warms up quickly. She is already spayed. Mitch has a heart murmur, is dehydrated and possibly hyperthyroidism. Owner said she had chronic vomiting. Needs further medical eval and a new home. PLEASE GIVE THIS SWEET CAT A CHANCE FOR A HAPPY ENDING – RESERVE HER BY NOON!!

ACTIVITY LEVEL: Laid back VOCAL: Quiet CHARACTER TYPE: Calm, Sweet MEDICAL BEHAVIOR: 8/29/17- Observed Behavior – good appetite, shy initially but warms up very quickly and loves getting pet; purring very loudly EVALUATION: Cage Condition: No change Reaction to assessor: Mitch immediately comes soliciting at the front of the cage. Reaction to door opening: Mitch remains standing at the front of the cage, relaxed body posture, soliciting attention. Reaction to touch: Mitch head-butts the assessor’s hand and appreciates petting on the head and body. Reaction to Being Picked up: Allows the pickup and remains calm. BEHAVIOR SUMMARY: Beginner Mitch interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.

History : came in as a euth request because owner reports she has been vomiting chronically and owner is unable to pursue further medical treatment; owner eventually surrendered her to ACC for further evaluation Subjective: QAR Observed Behavior – good appetite, shy initially but warms up very quickly and loves getting pet; purring very loudly Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 3/9 EENT: Eyes clear, ears clean, no nasal discharge noted; suspect thyroid slip Oral Exam: mm pk, moist; CRT <2 sec; moderate to severe tartar, staining; halitosis PLN: No enlargements noted H/L: grade III/VI heart murmur, eupnic ABD: soft, nonpainful, doughy U/G: female; reported to be spayed MSI: ambulatory x 4 with no lameness noted; hair coat slightly unthrifty; moderate diffuse muscle wasting CNS: mentation appropriate – no signs of neurologic abnormalities Rectal:not performed Assessment H/o chronic vomiting Diffuse muscle wasting Heart murmur Possible thyroid slip Dental disease Plan Rec’d CBC/chem/T4, currently out of stock of T4 Prognosis: fair to good; signs appear consistent with hyperthyroidism which can in most cases be well managed and has a good prognosis; if T4 noted to be WNL, then further diagnostics such as abdominal ultrasound should be pursued to identify the underlying cause of chronic vomiting/muscle wasting SURGERY: spayed

Hx: Recent hx vomiting and lethargy. Emaciated, suspect hyperthyroidism. S: Calm and alert in kennel. Allows handling. No purring this morning. O: BAR, estimated 8% dehydrated based on decreased skin turgor. BCS 2/9, MMs pale pink with thick stringy saliva. EENT: No discharge OU, mild black waxy debris on AU pinna (not in canals). Moderate tartar. PLNs: Not significantly enlarged. H/L: Grade 3/6 systolic heart murmur and possible gallop rhythm. Eupnic, quiet lung sounds. Abd: Tucked up, soft, no pain on palpation, no masses palpated M/S/I: Amb x4. Generalized moderate muscle wasting. No skin lesions noted. UG: Female reported spayed Neuro: Alert and appropriate, no sign neurological deficiencies A: 1. Dehydration 2. Heart murmur 3. Emaciation 4. Hx vomiting R/O hyperthyroidism vs. lymphoma vs. other conditions Short-term prognosis: Fair P: 1. LRS 200 ml SQ SID x5 days 2. Recommend placement with T4 testing ASAP, start methimazole if appropriate. 1088
