[DVM Intake] DVM Intake Exam felv+ elisa Estimated age:1 year Microchip noted on Intake?n Microchip Number (If Applicable): History 😮 died Subjective: Observed Behavior -calm , allows exam Evidence of Cruelty seen -n Evidence of Trauma seen -n Objective T = P =200 R =wnl BCS 5/9 EENT: Eyes clear, ears mites +, no nasal or ocular discharge noted Oral Exam:missing several teeth, forls PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G:2 testes MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: dental dz, ear mites, felv+ Prognosis:guarded long term Plan:dental in future, ivermectin SURGERY: Okay for surgery

ENRICHMENT NOTES:: 1/4/18 Curled up in litter box, eyes dilated. Lay in place when door opened. Hissed when bedding moved out of the way. Continued hissing when attempted touch, put ears sideways, backed into corner.
