Medical Behavior Evaluation BLUE
Medical Summary 05/12/17 17:04 “Missie” has shown mild improvement from her previous exam and seems to have a little more energy, willingness to eat and alertness. I recommend to continue supportive care and monitor her regularly over the evening and night…and seek NH placement. If her condition does not improve or deteriorates I recommend that she be humanely euthanized. – 0577 DVM Intake Exam Estimated age: 3-4 weeks Microchip noted on Intake? scan negative History : found stray (5 kittens total, 2 DOA) Subjective: Dull, depressed, dehydrated – more alert than littermate Observed Behavior – dull Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = 84 P = 96 R = 36 BCS 2/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: nsf, deciduous incisors and canines erupting PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; * difficult to auscult kitten ABD: Non painful, no masses palpated U/G: semi-solid stool, urinated MSI: poor BCS – mod/severe dehydration – moderately depressed behav.; able to move all limbs, no obvious signs of trauma; skin free of parasites, no masses noted, healthy hair coat CNS: dull mentation likely due to dehydration/hypothermia (+/- hypoglycemia?) – no other signs of neurologic abnormalities Rectal: nsf Intact female Assessment: – dull, depressed, dehydrated, hypothermic but more alert than littermate – was able to force feed 4 ml of soft food slurry – Parvo snap test negative Plan: supportive care – gave 10 ml warm LRS SQ – placed on heating pad – cont to force feed and monitor condition **temp . vax waiver due to age/health – rec’ NH placement or EHR if no improvement **gave hour deadline for improvement Prognosis: guarded if does not respond to treatment – rec’ NH placement/foster SURGERY: Temporary waiver due to age and health
