Medical Summary DVM Intake Exam Estimated age: 8 weeks based on weight and dentition Microchip noted on Intake? Scanned negative History : 2 stray kittens – were being fed by someone until she brought them here Subjective: Purring, allows all handling Objective BAR-H, MMs pink and moist, BCS 5/9 EENT: Moderate purulent nasal and ocular discharge. OU also chemosis and conjunctival erythema. Oral Exam: All deciduous teeth PLN: No enlargements noted H/L: NSR, NMA Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female intact kitten MSI: Flea dirt. Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment: Conjunctivitis and URI Plan: 1. Erythromycin OU BID x10 days 2. Amoxiclav 0.25 ml PO BID x10 days 3. Nebulize BID x5 days Prognosis: Good SURGERY: Temporary waiver due to CONJUNCTIVITIS and URI 1088
