Medical Summary DVM Intake Exam Estimated age: 1yr Microchip noted on Intake? none History : owner surrender Subjective: Observed Behavior – very friendly, social, allows all handling, no aggression Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = 138 R = purring, but eupneic BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: scant gingivitis, pink gums PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI, no palpable MGTs, grossly normal MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, L lateral neck- two small areas of alopecia, no skin lesions- approx 2cm each, R lateral neck- scab, R lateral elbow- approx 6cm spot of alopecia CNS: mentation appropriate – no signs of neurologic abnormalities Assessment alopecic spots: r/o trauma, prev infection other FI Plan Wood’s lamp negative offered canned food- ate readily ok for spay and adoptions Prognosis: good SURGERY: Okay for surgery
