Medical Summary 03/28/17 DVM Intake Exam Findings History – Owner surrender, no medical concerns noted. Microchip scan neg. Subjective Observed Behavior – Shy, tense, hissing. Becomes more agitated with increased handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: QARH. mm=pink, moist, CRT<2 s. P = 180 R = 28 BCS = 6/9. ORAL: Minimal calculus. EENT: Eyes clear, ears clean, no nasal discharge noted. Left ear tipped. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic. ABD: Tense, unable to palpate deeply. U/G: Neutered male. MS: Ambulatory x 4, no lameness. INTEG: Full, clean haircoat. No skin lesions. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Apparently healthy adult cat. Age 3 yrs. P: Monitor while at BACC. Excellent prognosis.
