DVM Intake Exam Estimated age: reported to be 8, age consistent with exam Microchip noted on Intake? neg History : owner surrender Subjective: BAR Observed Behavior – growling, swatting lunging in cage; used thick gloves to handle for exam; growled and vocalized through entire exam/tech check Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 7/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, moist; CRT <2 sec; moderate tartar/staining PLN: No enlargements noted H/L: difficult to auscultate due to growling and BCS; no obvious mumur/arrhythmia; eupnic with normal lung sounds ABD: Non painful, no masses palpated U/G: spayed; suspect spay scar on abdomen MSI: Ambulatory x 4, skin free of parasites, no masses noted, mild dandruff over dorsum CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Obesity Plan Rec’d weight loss through dietary management Prognosis: good SURGERY: spayed
