MEDICAL:
02/02/17
S/O: QAR. ~5-7% dehydration. Large amount of normal urine on bedding. Eating well. Allows all handling
No other changes to physical exam — continues to stay mostly in lateral recumbency but able to stand and walk (weakly)
A: R/O nerve damage vs other, also on treatment for lower respiratory infection
P: Rec advanced imaging with placement. Scheduled for CBC/Chem. Open prognosis
02/01/17
Monitor condition, check spinal/pelvic radiographs
S/O: QAR. ~8-10% dehydrated. Interested in food, appetite OK but not as good as yesterday. Remains mostly in lateral recumbency, large amount of normal urine on bedding this AM, urinated in litter box in afternoon. No feces. Allows all handling, attention seeking.
HL: Slight increased respiratory effort when resting, lungs clear
MS: Mostly lateral recumbent but able to walk and stand
Spinal and pelvic radiographs: No spinal or pelvic fractures appreciated; possible compression of intervertebral space at caudal thoracic spine (slightly rotated on lateral view); mild bronchial lung pattern
A: R/O spinal nerve damage causing paresis; mild lower respiratory disease
P: Continue with LRS and simbadol, adding pradofloxacin 0.7ml PO q24 x 7 days. Good/fair short term prognosis; Open long term prognosis – unable to determine if or how much function will return with time and supportive care

EENT: Mild dental tartar, pale mm, no oral ulcers, eyes clear, no ocular or nasal discharge, ears wnl
HL: Normal thoracic auscultation, no murmurs/arrhythmias, lungs clear
ABD: No palpable masses or organomegally, small amount of formed feces in colon, small bladder
INTEG: unkempt dirty hair coat, generalized thin brittle coat with dandruff, large amount of flea dirt
MS: Ambulatory x 4, dorsal curvature of thoracolumbar spine, hunched under in hind end, soft tissue swelling in region of hips, no palpable fractures, no crepitus appreciated, painful on spinal palpation
UG: Intact male
A: Emaciated, dehydrated, spinal curvature and hind end lameness — rule out old injury, nerve damage
P: Rec LRS 100ml sq q24 x 3 days and simbadol 0.32ml sq q24 x 3 days . Scheduled for spinal radiographs. Fair/good prognosis depending on diagnostic results. Appears to be old injury
